AI in African health systems is set for a major expansion following a $50 million partnership between the Bill & Melinda Gates Foundation and OpenAI aimed at strengthening fragile healthcare delivery across the continent. The initiative, unveiled on Tuesday, will begin in Rwanda before scaling to other African countries in the coming years.

The programme, known as Horizon1000, targets 1,000 primary healthcare clinics and surrounding communities by 2028. It is designed to help countries facing severe shortages of doctors, nurses, and facilities use artificial intelligence to improve access to quality care.

Under the initiative, AI tools will be deployed to support frontline health workers rather than replace them. The focus will be on improving diagnostics, managing patient records, streamlining clinic operations, and enhancing overall efficiency in under-resourced health systems.

Bill Gates said artificial intelligence could play a critical role in expanding healthcare access where human resources are limited. He explained that the goal is to equip health workers with better tools to deliver faster, more accurate care, particularly in primary health settings.

OpenAI said it will provide technical expertise and AI infrastructure while prioritising ethical deployment. Officials involved stressed that close collaboration with governments and local health institutions will be essential to ensure the technology fits local realities and health needs.

The Horizon1000 rollout builds on the Gates Foundation’s long-standing investments in digital health across Africa, including electronic medical records, mobile health platforms, and AI-driven disease surveillance. Sub-Saharan Africa currently faces a shortfall of about six million trained health professionals, contributing to preventable deaths from infectious diseases, maternal complications, and childhood illnesses.

Rwanda was selected as the starting point due to its strong track record in health innovation. The country hosts an AI health centre in Kigali and already uses drone technology to deliver medical supplies to remote areas.

The partnership comes amid shrinking global aid budgets and rising pressure on health systems. In recent years, the Gates Foundation has expanded its focus on scalable solutions, including a $2.5 billion commitment to women’s health innovation and support for national AI hubs, such as Nigeria’s AI Scaling Hub launched in 2025.

As AI in African health systems gains momentum, Horizon1000 is expected to test tools for maternal care, disease detection, appointment scheduling, and health worker training, offering a blueprint for technology-driven healthcare reform across the continent.
